Soundwave has appeared in a total of 3 episodes. 2 of them with Laserbeak and Ratbat. Allthough the second time he appeared was a 2 part episode so you could count it as 2 for Soundwave and 1 for Laserbeak and Ratbat.
"Sound and Fury" where he appeared WITHOUT Laserbeak or Ratbat. I don't know how true this is but what I've read is that the show's animators and wrighters were not aware of Hasbro's desision to include Laserbeak with the toy otherwise he would of been included in this episode. Soundwave was destroyed at the end of the episode but they did show a dramatic close up of a G1 Nod tape recorder with the Decepticon symbol on it. For Soundwave sence he is not a true Cybertronian and wasn't powered by an AllSpark fragment that G1 fan nod seems to take the place of him haveing a spark sence it seems Soundwave can easily be rebuilt as long as that tape recorder is still in tact.
In the "Human Error" 2 parter the show made a second attempt at Soundwave and included both Laserbeak and Ratbat who technically comes with the Soundblaster repaint. Yeah I know it's not called Soundblaster but it's a Black Soundwave so that's what I'm going to call him because Electrostatic Soundwave is a supidly long name. Anyway, Ratbat and Soundwave were both destroyed at the end of the 2 parter. Laserbeak however got away and was seen at the end of the episode flying away with Soundwave's G1 tape recorder fan nod "Spark" thingy.
So the short answer to the first question is Soundwave never died but his body was destroyed twice. The short answer to the second question is Laserbeak and Ratbat appeared in "Human Error" they're only seen at the end of part 1 but they're throughout the episode in part 2.
I don't know if anyone has thought about it this way or not but looking at Soundwave's different forms, they each seem to represent something in a Transformer. He's got his transforming body that seems to be easily damaged and easily rebuilt/repaired due to the fact he somehow formed that body around his original design.
The original toy design for Soundwave that Megatron created seems to be a sort of Protoform. Not exactly liquid metal but in the sence that he doesn't have an alt mode yet and gains one later in the episode.
And finally the tape recorder seen at the end after his body is destroyed each time seems to represent his spark as it seems Soundwave can't truely be dead as long as it still exsists.
Animated Soundwave is more unique than G1 Starscream was made out to be in Beast Wars.
